Patient Registration

Registering with us is easy. Your registration form can either be completed online or you can collect a 'new patients' form from reception. You will need to come to the surgery for a new patient's medical check and bring your medical card to the surgery to complete the registration.

If you are changing your address or contact details then let the reception know as soon as possible.

All you need to know about registering online with Welland Medical Practice

At Welland Medical Practice we have devised a simple guide for easy registration with us. Please find the headings into which you fit below and follow the instructions for (hopefully) stress-free registration.


If you live within our practice area we are happy for you to register with us.

You do not have to go back to your existing GP surgery in order to register with us.

You can complete the paperwork online or print off a form and telephone the reception to book an appointment for a new patient check.

It is strongly recommended that you register with a GP in the town where you live, even if you already have a doctor somewhere else in the UK.

If you have never lived in the UK

You need to register with a Medical Centre in order to obtain a National Health Service (NHS) number. This allows you to:

  • Receive full NHS medical services whilst you are resident in the UK

  • NHS medical care is free and available throughout the UK to those who are resident in the UK for a period of six months or more.

If you have lived in the UK and were registered with a doctor here once upon a time but now live abroad you should already hold a National Heath Service (NHS) number. You are strongly recommended to register with us to receive full NHS services.

Registering with Welland Medical Practice.

Step 1

Complete the form below. Please ensure that you fill in all the fields and provide the information that is required.  

Step 2 

You will receive confirmation of your application by e-mail and details of how to arrange a short appointment to come in to the medical centre to meet a nurse and sign your form. This appointment could be booked on line or over the telephone.

Step 3

Bring some documents with you to the new patient medical appointment. We would like 2 proofs of your address; one should ideally have a photograph of you on it. If you have recently arrived from overseas please bring your passport/visa alongside the 2 proofs of your address.

A nurse will give you a brief medical examination. This will include simple things such as measuring your blood pressure and weight.

Step 4

You have now successfully registered. Congratulations! Your medical card, which formally shows that you are entitled to receive NHS medical care, will be sent to your address. It may take some time (snail mail).

Please be patient. If you change your address please let the medical practice know A.S.A.P.

Step 5

Welland Medical Practice will write to your previous GP, if you have one, and the records will be sent through to us. This can take a few weeks before we have your complete medical records. In some cases we can ask for this process to be speeded up. Please let us know if we are likely to need your full records urgently.

Thank you for completing this questionnaire. We invite all new patients to make an appointment with the nurse /HCA for a new patient health check and a NHS Health Check if your aged 40 – 74.

If you would like to do this please ask at reception. We request that you bring a urine sample to

your first appointment.
